A "continuity error" is what happens when something that should be the same changes between shots – such as the position of a prop or the colour of an item of clothing. They come about because films are not shot in chronological order, and sometimes scenes or specific camera angles have to be reshot after the principal photography has concluded. This requires the crew to painstakingly reassemble the visual composition of a scene exactly as it was days, weeks or even months earlier.
The many continuity errors in Apocalypse Now are likely the result of its famously complex and difficult production process. A typhoon destroyed sets in the Philippines mid-way through production, availability issues sometimes required different shots from the same scene to be filmed weeks apart, and the lead actor (Martin Sheen) suffered a near-fatal heart attack and had to be replaced with a double while he recovered.
