The first digital face replacement in a film occurred on the character Lex (Ariana Richards) in Jurassic Park (1993) and was orchestrated by Industrial Light & Magic (ILM). In the film, Lex falls through a ceiling panel while being chased by a velociraptor. Although the actual fall was done by a stuntwoman, Richards' face was added into the shot via digital face replacement by ILM, the first time it had ever been done.
