The most swimming medals won at the Summer Paralympics by an individual male is 27, achieved by Daniel Dias (Brazil) between 2008 and 2021.

Dias won three bronze medals on 25–26 August 2021 at the Tokyo 2020 Paralympic Games to take his career tally to 27. He won 14 golds, seven silvers and six bronze. Born with impairments to his upper and lower limbs, Dias didn't learn to swim until he was 16.
